You might try asking your question in our (Netscape's) public support
newsgroup, which is accessible via:

I can't follow your description well enough to understand what problem
you are having.  If you use type=checkbox within a Netscape Directory
Server Gateway template file, an HTML checkbox form element is
generated.  Is that not what you want?
