You most likely have a duplicate olcPasswordHash entry. Due to a quirk, it can be set on the frontend and global config. Ensure it is ONLY set on the frontend and remove it from the global config.
This does not apply to olcPasswordCryptSaltFormat, which MUST be set globally.
