$ rpm -qf /usr/lib/openldap2.3/ppolicy.la
openldap2.3-servers-2.3.20-1.rhel4es

Most likely your modulepath is wrong:

# grep modulepath /etc/openldap2.3/slapd.conf
modulepath      /usr/lib/openldap2.3

'rpm -ql openldap2.3-servers' would probably help?
