Yes that helped.  It appears that I was doing a couple
of things wrong -

1. Don't need to put the moduleload directives in
slapd.conf:
modulepath ../servers/slapd/overlays
moduleload ppolicy.la

2. I was missing a directive:
ppolicy_use_lockout
