To start with, set the log level to 776 (conns + stats + stats2)
and restart slapd. Try to connect using your PHP code and
send whatever appears in the log (probably /var/log/messages)
Send also a copy of the slapd config with passwords removed,
and let us know what errors the PHP code reported.
