Yes, that would work. You don't need to provide the {x} prefixes unless you're adding in a non-sequential order.

Possible. You replace all values with first one, then adding rest
values, like this:

dn: olcDatabase={2}hdb,cn=config
changetype: modify
replace: olcAccess
olcAccess: {0}to dn.base=""  attrs=namingContexts  by * none
-
add: olcAccess
olcAccess: {1}to * by * read
-

There's no need to break it up that way.
