Is it possible to do inserts with back-config?

Yes. See http://www.openldap.org/doc/admin23/slapdconf2.html section 5.3.6 "Access Control Ordering"
Also see http://www.highlandsun.com/hyc/drafts/draft-chu-ldap-xordered-xx.html

I mainly ask because currently I have a very large number of ACLs (800+ lines), and one of the things that concerns me about converting to back-config is when I have to add new ACLs. Essentially I want to be able to add a new ACL somewhere between 2 existing ACLs, like if I had:

olcAccess: {210}....
olcAccess: {211}....

I may need a new ACL that comes between 210 and 211, and I don't want to have to manually renumber everything.
