I've seen problems when the database directory used during testing is on an NFS mount (may have been due to the fact that nfslock was not running or similar, though I could not check). Note that it would be more useful to give the exact error message returned by slapd, which will be in the logs provided (tests/testrun/slapd.1.log?).
