As a resolution to this post i thought i'd update the list on how we
fixed this problem.

We decided to upgrade all our servers to OpenLDAP 2.2.24 with BDB
4.2.52 + recommended patches and it seems like the issue has gone
away.

In short BDB 4.2.52 + patches.... good and BDB 4.3.21....... bad :o)
