i stopped slapd and ran db_recover, ran slapindex, and then started slapd once again. is that the correct sequence to follow? i'm still not seeing any sort of performance improvement.

i'm honestly not sure how relevant this is but, if i look at the
output of "db_stat -m", it appears that only about 260KB is being used
for cache space (output trimmed):


Since the BDB default cache size is 256KB it appears that your setting has not taken effect. And judging from the db_stat output "pages forced from the cache" this default cache size is too small for your database. I suggest you reread the FAQ and/or the BDB documentation for information on how to correctly set the BDB cache size.
