Our setup is as follows:

OpenLDAP 2.1.22 + BerkeleyDB 4.1.25
Redhat 9 (2.4.20-20.8smp)
4 2.8GHz Xeon processors
ext2 filesystem w/ RAID mirroring

I'll suggest 2 things:

1) Upgrade your version of OpenLDAP to 2.1.25 (There were serious bugs in 2.1.22)

2) Upgrade your version of BDB. You should use BDB 4.1.25 + patch ( at a *minimum*, and preferably, (4.2.52 plus patch).
