Thanks Chris, I added objectclass: puser to the above entry
and used ldif2ldbm to recreate the databases. 
However, whilst trying to add a user with PHP I get the same error

  > and the error log shows
  > LDAP: No such object
  > LDAP: matched: provider=bar
