HEADS UP: LDAPMod/Friendly changed and more

The mod_next field of this structure has been axed to conform
with the IETF draft API spec.  It is likely only slapd used
this field, but...

Also, Friendly has been renamed LDAPFriendly to fit within the
LDAP namespace.  It's field members names have also been changed.
This struct is not widely used, but...

The next pending API change moving LDAPServer, LDAPConn, LDAPRequest,
and LDAPCache to ldap-int.h.
