I detect an intriguing difference in our configurations.

I was misled by this comment in SampleLDAP.pm:

# Usage: Add something like this to slapd.conf:
#
#       database        perl
#       suffix          "o=AnyOrg,c=US"
#       perlModulePath  /path/to/this/file
#       perlModule      SampleLDAP

The man page is more accurate.  This was apparently actually a minor bug
in the comments.  :)
