What is the root cause of the connection failure?  This is essential to
reproduce the issue and understand what is going on with the proxy's
code when retrying a broken connection.  As I stated during ITS#4409
investigation and resolution, I've tried many different failure cases,
and the only hosed code I found was related to concurrent tries to
implicitly bind a newly created connection.  I'm not even sure this
would possibly occur for real, as I had to force it from inside the
debugger.
