The slapd server refused to die after this point.  I had to kill -9 the 
server.  Once that had been done, I did a db_recover and restarted slapd. 
Slapd now reported that the entry I had added didn't exist, so I assume it 
failed to write the entry out to the DB.

Adding the entry again failed once more, hanging forever.

As far as I can tell, it is essentially impossible to add entries to a 
master that has syncRepl replica's attached to it under 2.2.11.
