slapd exiting with signal 6 and out of locker entries (ITS#2678)

slapd process keeps crashing with the following error message:
"pid 91207 (slapd), uid 1000: exited on signal 6"

Berkely DB version 4.1.25_1.  Configured as slave in a replication agreement
with another openldap server.

Restarting slapd succeeded for several days.

now starting using "slapd -d 1024" produces the following
