On the master, in the accesslog overlay section for the main DB, you're 
missing the following parameter:

logsuccess      TRUE


On the master, in the accesslog database section, you are missing the 
syncprov overlay:

overlay syncprov
syncprov-nopresent  TRUE
syncprov-reloadhint TRUE
