Here is what I have

access  to *
  by self write
  by peername=10.19.14.141 write

I have also converted it to slapd.d directory with same results.

What log files would you recommend?  I have tried debug 255 which I can port.

Yes, Version 2.0 slapcat.
Yes, Version 2.3 slapadd and it was created without error.
