I think index isn't created correctly or it is damaged.
My version of openldap is 2.2.23-8 and OS is Debian.
That version sounds a bit old.
Perhaps someone may help you with a bit more information, for example:
- backend type?
- database version/patches?
- slapd.conf file (index section)?
- attr definition/syntax?
- what did the log say?
