index broken? server cannot lookup anything

hi all,

i noticed a really strange behavior of my ldap server this morning. the
server cannot lookup anything anymore. the error just occured without
any manual changes on the configuration or index. i run 2.1.30 with
berkeley db 4.1.25.

i tried to do some manual updates and got his on the cli:

ldapmodify: update failed:
cn=1218,cn=SMSGW,ou=portals,cn=3000740,cn=netm,ou=resellers,ou=netmservice,o=netmldap
ldap_modify: Internal (implementation specific) error (80)
        additional info: id2entry failure

this is what i can see in the logfile of slapd:

Mar  8 09:00:44 hestia slapd[19642]: conn=261 op=7988 SRCH
base="cn=1218,cn=SMSGW,ou=portals,cn=3000740,cn=netm,ou=resellers,ou=netmservice,o=netmldap" scope=0 filter="(objectClass=*)"
Mar  8 09:00:44 hestia slapd[19642]: conn=261 op=7988 SRCH attr=*
Mar 8 09:00:44 hestia slapd[19642]: ldbm: ==> fatal region error
detected; run recovery
Mar  8 09:00:44 hestia slapd[19642]: conn=261 op=7988 RESULT tag=101
err=32 text=

any suggestions? i don't know what exactly happend here... could it be
that the index is somehow "broken"?

thanx for your help,
carsten
