ldbm index of otherMailbox and approx for telephoneNumber

Converting to OpenLDAP from really old product called IDDS from Innosoft.

OpenLDAP using LDMB/gdbm, it doesn't want to do any sort of indexing on
otherMailbox (was doing eq,approx,sub on old server), and complains if I try
to do approx indexing on telephoneNumber (eq and sub are OK).
