That's a problem right there. OpenLDAP 2.0.x is deprecated and no longer supported. I strongly advice upgrading to a recent, supported version of OpenLDAP, like OpenLDAP 2.2.13.

  index   mail,cn,mid eq,subinitial

Hm, in 2.2, it is "sub", not subinitial for substring?
