I'll suggest 2 things:

1) Upgrade your version of OpenLDAP to 2.1.25 (There were serious bugs in 2.1.22)

2) Upgrade your version of BDB. You should use BDB 4.1.25 + patch (4.1.25.1) at a *minimum*, and preferably, 4.2.52.1 (4.2.52 plus patch).
