Its really not telling me anything else.

Has anyone seen this before?  Shoould the file be created before? Is it
looking for certain permissions?

In 2.1.x, it is slapcat -l <filename> to dump your database. Not sure what you are trying to do here.

For the slapd loglevel bit, that was for 2.1.x. I'm not sure if that will work the same in 2.0.x.
