Try this in the schema, I got this too, your trying to have
multiple attributes with different values?   Hope this helps.

Example:

attributetype( 1.2.3.4
  NAME 'nisnetgrouptriple'
  DESC 'I can exists many times in a single entry'

  EQUALITY objectIdentifierMatch
  ORDERING caseIgnoreOrderingMatch
  SYNTAX 1.3.6.1.4.1.1466.115.121.1.40 COLLECTIVE )
