Can you try adding without this entry?
I had a similar problem on RHL 6.2, the base dn cannot be added to the
db. I managed to add entries below this though.
i.e., if your base dn is 
dn: dc=example,dc=com in slapd.conf, then you cannot add it to the db.
However dn: dc=foo,dc=example,dc=com can be added.
