Try using 
ldapadd -D "cn=admin,dc=beth,dc=k12,dc=pa,dc=us" -w
mysecret -f /etc/openldap/hierarchy.ldif.

Important check for one more thing that to add an
entry like user=xx,dc=yy,dc=us, the entry dc=yy,dc=us
must already exist. Otherwise usually it will give an
error ldap_bind: no such object
