At which point you should have run database recovery.

Upgrade to 2.3.x to avoid this happening in future (or call 
slapd_db_recover -h /var/lib/ldap in some script that runs at startup, or in 
the start() function in the init script).

slapcat, slapadd. But, then you may as well upgrade to 2.3, and get new 
features too (such as smbk5pwd overlay which may help you with password 
synchronisation etc.).
