I've set up a test where a proxy is contacted much like the way you
indicated, i.e. the same connection is used for repeated binds with
different identities.  I force connections to timeout very often by way
of the idle-timeout directive, but I couldn't see any crash.  I really
think you should provide:

- a log of your proxy at "stats" level, to see the actual sequence of
operations that eventually results in a crash
- a gdb stack backtrace of a crashed proxy; I might ask you to print
selected values from that stack, so keep the binary and the core around
for a while.

The fact that the proxy crashes in about 5 minutes should make the
collection of this info very easy to produce.  The fact that it crashes
in an assertion that I haven't been able to reproduce seems to indicate
that the proxy is doing something unexpected that hasn't been tested so
far.
