The slap tools don't call lutil_passwd_init by themselves, it's supposed to be
taken care of automatically. However, if a dynamic password module is loaded and
calls lutil_passwd_add in a slap tool, lutil_passwd_init doesn't get called
first so only the loaded password mechanism is defined. The regular compiled-in
mechs never appear.
