Total operations missing from monitor backend (ITS#2983)

In 2.1, there was a "cn=completed,cn=operations,cn=monitor" entry, that would
tell you the total number of completed operations (ADD, MOD, SEARCH, etc).  This
no longer exists in 2.2.

It seems to me that the "cn=operations,cn=monitor" entry is missing two
operational attributes:

monitorOpInitiated
monitorOpCompleted
